Welcome to the Oakland Police Department's Medical Resource Site. Our goal is to provide as much support to the Police Department 24/7 as possible.
We currently have all forms required for Declinations, Initial Injury Reports (Worker's Compensation Claim), requesting Transitional Assignment employees, and Family Medical Leave available electronically.
Some of the forms are available in Adobe Acrobat and require Adobe Acrobat Reader, which is available as a free download from www.adobe.com.
Coming soon are training guides, weekly reports, and policies and procedures.
